Aleksandra Pikula, MD — vascular neurologist, Professional Coach in Life and Wellness, and founder of HER PRISM
Aleksandra Pikula, MD

Physician-coach. Brain-health scientist. The voice on the other side of this work.

Through HER PRISM™ I coach women in medicine — physicians, surgeons, healthcare providers, those still inside academic medicine and those quietly leaving it — globally and online. The work is coaching, not medical care. My clinical training is the lens I see through, not the service I deliver.

HER PRISM™ came out of my own recovery from burnout — and then out of the long, unglamorous years of testing it, refining it, and writing it down so that it could become something other women in medicine could actually use.

My role here is not content creator. Not influencer. I am the architect of a new conversation — about brain health, midlife, and what sustainable leadership looks like for women in medicine.

Outside the clinic

A little of who I am.

I am originally from Croatia and now live in Toronto with my family and our cheerful labradoodle. When I am not in the clinic, you will find me cooking, hiking, travelling, swimming, practicing yoga and meditation — or doing what I believe matters most: building communities rooted in connection and well-being.

I love narrative and reflective writing, biographies, and non-fiction — and exploring how women's health is understood across different cultures.

The story behind HER PRISM

What 20 Years in Vascular Neurology Taught Me About My Own Brain.

Twenty years in vascular neurology. Ten years studying women’s brain health. I knew the data. I taught the data. I wrote guidelines, and the papers on what chronic stress and mediocre lifestyle does to a midlife brain.

And I was sleeping five hours a night. Running on cortisol. Working seventy-plus hours a week. Advocating for balance in lectures while my own balance was collapsing inside me, quietly, like a slow leak nobody could see. I was the Vascular Neurologist teaching and translating a science I had refused to receive.

Then the body told the truth before I would. The 3:47 a.m. wake-ups. The brain fog during clinical decisions. The hot flashes during a presentation in front of the department I had spent two decades earning my place inside — feeling, even then, even more as an imposter. Perimenopause colliding with twenty years of accumulated allostatic load, layered on top of a career that had been quietly extracting more than it gave back for a very long time.

What I did next — the sixteen weeks I built for myself, the travel, the coaching and the books and the framework that finally held — became the foundation of HER PRISM™.

The full story — including the hard truth about who did not want me to heal, and the terms I came back on — is the origin essay of HER PRISM™, published on Through HER Prism.

What sits underneath the method.

Philosophy

What you can expect from this work.

HER PRISM™ is direct, intentional, evidence-forward, and unsentimental about the systems that have depleted you. It is also warm. It treats you as an adult clinician — not a patient, not a project — and it expects you to do real work. It does not promise softness; it promises science, structure, a return to your authentic values, and a documented before-and-after. HER Bio-Harmony™ by design.

It is also personally calibrated. The framework — the five principles, the validated instruments, the sixteen-week arc — does not move. How it shows up in your life, at your pace, around your nervous-system bandwidth and your clinical schedule, does. Fidelity to principles, adaptation to the actual woman in front of us. The shape is mine. The life inside it is yours. We do the work together.

It also keeps a clean line. Nothing about HER PRISM™ is connected to or affiliated with any institutional appointment, university, hospital, or research chair. The brand stands on its own clinical and academic foundation.
